On August 26, 2021 the Ministry of Health released new screening questions for patients. (For in-person appointments only of course.) This COVID-19 Patient Screening Guidance Document was updated to reflect screening for fully immunized individuals, re-orders the patient questions and updates the symptom list. OAND staff have incorporated this new guidance into a revised COVID-19 Screening Questions for Patients Information Sheet/Poster for members to post and the Screening Questionnaire for patients to fill out. They can both be found below and we hope that they prove helpful for members to use:
